ON LINE REGISTRATION FORM

The Players Club at Tahquitz Creek offers golfers the following benefits for the period of 1 year ("Term") (3 month term also available below). In consideration for the prepaid fee, the Player and any included family members listed below shall have the following privileges:

(i) Range practice, to include 1 Large bucket of range balls available any time of day, 7 days per week, provided there is good weather at the club and the driving range is open.

(ii) Instruction privleges during the scheduled clinic times at the range

(iii) Free greens fess + $10 Legend / $15 Resort cart fee access during specified Players Club Access times (see below)

(iv) up to 3 day advance reservations

(v) Players Club Corner (select merchandise available for purchase at cost)

(vi) 20% Discount on purchase of range keys

The Players Club is available for the Player and, for an additional fee, up to two (2) of the Player's family members. Family members include spouse and children living in the same home. Upon execution of the Agreement and payment of the Plan Fee indicated below, the Player and named family members shall receive all Plan benefits and privileges

Single Legend Course Only - $290 (Annual Fee) - $29 (Monthly Fee + BONUS 2 Months Free)
Single Legend & Resort - $490 (Annual Fee) - $49 (Monthly Fee + BONUS 2 Months Free)
1 Additional Family Member Legend Course Only - $480 (Annual Fee) - $48 (Monthly Fee + BONUS 2 Months Free)
2 Additional Family Members Legend Course Only - $670 (Annual Fee) - $67 (Monthly Fee + BONUS 2 Months Free)
1 Additional Family Member Legend & Resort - $680 (Annual Fee) - $68 (Monthly Fee + BONUS 2 Months Free)
2 Additional Family Members Legend & Resort - $870 (Annual Fee) - $87 (Monthly Fee + BONUS 2 Months Free)

Annual Payment Option

I wish to pay my Plan Fees for the next year upon execution of this Agreement and understand there will be no refunds if I terminate my agreement prior to the term of the agreement for any reason. By checking this option I understand my credit card will be charged the full annual fee upon reciept of this agreement.

Monthly Payment Option

I wish to pay my Plan Fees in monthly installments to be due the fifth day of every month of the term. Club is here by auhtorized to make a charge to my credit card (credit card information will be provided at sign up). By checking this box I agree to the terms of this agreement and understand that my plan fees will be charged on a monthly basis at the rate selected above.

ACKNOWLEDGEMENT

CREDIT CARD ACKNOWLEGEMENT - By checking this box I agree to report to Club all changes to my credit card information within ten (10) days of the change. I agree that Club is not responsible for any charges or expenses (e.g. for overdrawn accounts, exceeding credit card limits, etc.) resulting from charges billed by Club. I agree to pay the charges by the due date and understand that I will forfeit my privileges and be unable to sign up for the installment payment plan in the future should an installment payment not be made for any reason.
PROGRAM RULES ACKNOWLEGEMENT - By checking this box I understand that The Players Club Program benefits or rates may be changed or terminated by the club at any time with 30 days notice. This notice is to be posted in the golf shop and at the practice facility for 30 days prior to any changes. This program is for the sole use of the member and any added family members listed above. Any member found giving range balls to nonmembers or removing range balls from the facility will be immediately expelled from the club without refund.

We will contact you in the next 24 to 48 hours to verify the credit card you wish to use for your membership. Verification of the credit card and photo ID will be required when picking up your membership card.
